Effective mode in responses
When telemetry is true, notre.mode in the response reflects the effective public mode. Some deployments may run shadow evaluation; the public contract documents shadow only when that deployment mode is active.
Fallback
If Notre errors or declines to intervene, the gateway forwards the original request. fallback_used in telemetry indicates that safe path was taken.
